Ecumen Pathstone Living is a non-profit, spiritual based community that has been caring for older adults in the Greater Mankato community since 1937. We provide a wide spectrum of services which include: Long Term Care/Skilled Nursing; Short Term/Rehabilitation Care; Assisted Living Apartments; Memory Care/Nationally Awarded Awakenings Program; Home Care Adult Day Services; Medical Equipment & Supply Store; Catering Services/Public Cafe. Ecumen Pathstone Living's foundation is built on its people. Dedicated team members, empowered residents, supportive families, caring volunteers, and devoted friends all contribute to the vibrant community of Pathstone Living. More than 500 people benefit directly from Pathstone services on a daily basis. Ecumen Pathstone Living is owned by Ecumen, the most innovative leader in senior services.

The Freedom Home Care team is passionate about serving others. It's our goal to care for the whole person with our home care and case management services.
Home Care
-Companionship - from socializing to providing transportation to activities
-Wellness and safety - including meal preparation, diet and nutrition, and personal safety monitoring
-Help in and around the house - Including grocery shopping, running errands, light housekeeping, laundry
-Activities of daily living - assistance with bathing, dressing, personal hygiene, mobility, toileting
-Management of chronic conditions - medication reminders, daily health monitoring and regular reporting of any changes
-Transitional Care - from skilled nursing facility or hospital to home
-Peace of mind - serving as an extra set of "eyes and ears" for families
Nursing Services
-Medication management - making sure our clients are taking their prescriptions and other drug treatments correctly, setting up their medications on a regular basis
-Hands-on assistance with transfers and mobility - training caregivers in transfer techniques to help our clients move about their home environment to reduce the risk of falls
-Delegate nursing tasks to caregivers - training caregivers on the skills needed to perform tasks related to a client’s unique care needs
-Coordination of Care - coordinating appointments, attending appointments, providing open communication between our approved members of the client’s care team
-Regular Monitoring Visits - noting any changes to the client's condition and making adjustments to ensure the success of the client's plan of care
